2. We fill out a health questionaire and determine if botox is suitable for you.

3. Questionaire is sent to our doctor, who will phone you to clarify any points if deemed necessary.

4. Botox is prescribed for you and practitioner  visits you at home to perform the treatment.

5. Follow up appointment two weeks to check on results.
